Description: It is well recognized that early diagnosis and ongoing management of Alzheimer’s Disease (AD) requires a patient-centric, highly coordinated effort among a large multidisciplinary team of clinicians. As such, this proposed initiative establishes meaningful, small group interactions among key stakeholders (neurologists, dementia specialists, geriatricians, geriatric psychologists, radiologists, and primary care clinicians) that will, through a variety of collaborative educational experiences, help all members of the care team consider approaches to reducing barriers to effective coordinated care and encourages each participant to create an action plan to improve the quality of AD care delivered within their organization. Considering the complexity of this challenge, lack of expertise, and the breadth of communication and cooperation required, participants would highly benefit from an educational intervention that includes discussions with knowledgeable experts and peers from within and outside of their own communities who can share experiences, strategies, challenges, and success stories.

Cassandra Vonnes

Geriatric Oncology NICHE Coordinator

Dr. Vonnes attended the University of South Florida for her BS, MS and DNP. Her 25 year NP experiences include acute care and outpatient nephrology and cardiovascular medicine. She was inducted as a Fellow of the American Heart Association for her contributions in cardiovascular medicine. Dr. Vonnes has taught clinical and didactic at the USF College of Nursing for undergraduate and Nurse Practitioner students. She was recognized as a Hartford Distinguished Educator Geriatric Nursing Her recent venture into oncology provides an opportunity to use her geriatric expertise as the Nurses Improving the Care of Healthsystem Elders (NICHE) coordinator at Moffitt Cancer Center achieving Exemplar Status. Under Dr. Vonnes leadership Moffitt Cancer Center was the first hospital in Florida to be recognized by the IHI and John A. Hartford Foundation as Committed to Care Excellence for the Older Adult. She has presented both nationally and internationally on Fall and Injury prevention, delirium, and geriatric assessment. Dr. Vonnes is the host of the podcast GAPNA CHAT that features leaders in Geriatric Health Care. The Gerontological Advanced Practice Nurses Association awarded her the Excellence in Leadership for 2022.
